Menopause Myths vs. Facts: Timing of Hormone Therapy
As a certified menopause specialist, I field a lot of questions about menopause and menopausal hormone therapy (MHT). Honestly, I welcome them - because few areas of women's health are more clouded by outdated information than this one.
Much of what women have been told about hormone therapy traces back to early interpretations of a single study that have since been re-examined and, in many cases, overturned. Science has moved forward - in fact, in late 2025, the FDA began removing the long-standing "boxed warning" from hormone therapy labels. Yet the myths linger - and they keep women from treatments that could genuinely improve, and in some cases protect, their health.
This series is designed to clear up these myths. Today I’m focusing on the TIMING of menopausal hormone therapy (MHT).
Myth: I have to wait until a full year has passed since my last period to start MHT.
Fact: Many women benefit from starting MHT during perimenopause.
You don't have to white-knuckle your way through the transition. Perimenopause - the years leading up to your final period - is often when estrogen and progesterone levels swing the most dramatically, driving symptoms like hot flashes, sleep disruption, mood changes, and irregular cycles. For many women, beginning MHT during this window can ease those symptoms long before the "official" one-year mark.
Myth: I have to start MHT very early in menopause, or I won’t get any benefit.
Fact: The "window of opportunity" is a full decade - not a narrow few months.
Current evidence points to a "window of opportunity" - generally the first 10 years after menopause, or before about age 60 - during which the benefits of MHT most clearly outweigh the risks for healthy women. Starting within that window is associated with the most favorable overall risk-benefit balance, including benefits for bone health and, for many women, the cardiovascular system. The takeaway is that you have a generous, decade-long runway, and beginning earlier in that span is generally more advantageous than waiting many years.
Myth: Women over the age of 60 are too old to take MHT.
Fact: There is no defined age limit for MHT.
The window of opportunity tells us when MHT offers the most benefit - but it's often misread as a hard expiration date, and that's where this myth comes from. The age of 60 gets referenced as a cutoff, but it's a fairly arbitrary line, not a strict scientific rule. While most women see the greatest benefit from starting within that first decade, the decision to continue MHT beyond 60 is an individual one, made between each woman and her provider based on her own symptoms, health history, and goals. It’s important to note that using MHT beyond the age of 60 continues the bone-health benefits achieved at younger ages, which are lost after discontinuing HRT, and especially important to be aware that vaginal estrogen, which can be used by postmenopausal women of any age is a true game-changer in preventing urinary tract infections - which, in elderly women, can become serious and even life-threatening (more on this in an upcoming blog.)
I'm hopeful that as more research emerges, we'll have even richer data on MHT in women over 60.
The Bottom Line
When it comes to MHT, timing matters - but there's rarely a single perfect moment, and far less often a moment that's truly "too late." Whether you're in the thick of perimenopause, freshly past your final period, or well into your 60s, the right answer comes from your own symptoms, health history, and goals - not from an arbitrary cutoff. Menopause is a stage of life every woman who lives long enough will reach, and it deserves care rooted in current science, not decades-old fear.
